Community Outreach Programs

 

The VNA Foundation and the Susan G. Komen Breast Cancer Foundation

Grants from the VNA and Komen foundations fund breast cancer awareness workshops in Chinese to uninsured Asian women in the community. After each workshop, women are linked to the Family Planning Program for clinical breast exams. Uninsured women are also linked to the IBCCP Clinic for free mammograms and diagnostic follow-up services. The highly trained medical staff speaks English, Spanish, Mandarin and Cantonese.

Prostate & Testicular Cancer Prevention Program

Funded through the IDPH Men’s Health Initiative, education & PSA testing are provided to increase awareness and reduce incidence of testicular and prostate cancer. These workshops are held at local health fairs, churches, senior centers and other community centers. Staff nurses and phlebotomists coordinate the PSA blood draw and participants receive a copy of their results within one week.

Asian Health Outreach

Networking efforts in Asian communities including Chinatown, Tinley Park, Naperville and northwest Indiana has made an impact in addressing the reproductive care in this underserved population. Community Health Outreach, MetroSouth’s OB/GYN Clinic, Family Planning, and IBCCP clinics have joined forces to create the Asian Health Initiative in which culturally specific outreach services are provided in the area of cervical cancer, breast cancer, prostate cancer, well baby care, prenatal care, doula services, and family planning services. In addition, translation services, case management and navigating patients through the hospital system are provided on an individual basis.
